What Tools Do I Need for Long-Term Self-Management?

Once patients are discharged from an episode of care, they usually will review a self-management program to be performed at home. Self-management programs are crucial for patients to continue to progress and reach their respective goals. For patients to be successful, clinicians need to understand how to utilize an individualized self-management approach1. The clinician needs to design a plan targeting biomechanical, psychological, and individual characteristics1.

The main goal is to make the patient an active participant in the process.  We need to implement strategies for patients to be successful with their long-term self-management programs.

Also, we need to help guide patients to discover what products are available to help facilitate continual growth and improvement.
